Urethroplasty with buccal mucosa grafting is quite easily performed in the case of penile stenosis. The simplest approach is the coronal approach but this is limited to stenoses located downstream of the peno-bulbar angle. If the stenosis is higher, the Kulkarni perineal approach, as described in Course 2, should be chosen.
The technique is based on the removal (like a tyre) of the spongiosa, which is separated from the cavernosum while keeping a vascular hinge on one side. A cavernous gutter is obtained at the bottom of which the mucosal graft will be fixed. The stenotic urethra in an inverted U shape will be sutured to the two edges of the fixed graft.
